It is hard for most of us to think or even talk about end-of-life. In reality, avoiding difficult conversations may make caregiving situations tougher for people we love.
Advance Care Planning (ACP) is a series of conversations about your wishes and preference before a serious or life-limiting illness strikes, and ensuring your wishes are communicated to people you love.
In this talk, understand why is it important to do ACP, its process and how to start tough conversations off on the right foot!
