Hospital-To-Work Programme

Overview

After a life-altering event, persons with acquired disabilities often have to re-assess their career trajectory and explore new job opportunities due to changes in their abilities. The Hospital-to-Work (H2W) Programme supports persons who have acquired disabilities, such as from medical conditions or accidents, in their transition to work.

Jobseekers in the programme can get personalised career re-assessment, rehabilitation and job-matching services. Persons with disabilities who are already in employment can also benefit from job support services. Ultimately, together with our partners, ABLE and SPD, we aim to bridge the gap between persons with acquired disabilities and employers to create a disability-inclusive culture in every workplace.

*Note: The type of support provided will be customised for each individual depending on his/her needs.


Who Can Apply

  • Singapore Citizen or Singapore Permanent Resident
  • Above 16 years of age
  • Medically certified as fit for employment
  • Persons with a permanent acquired disability*^ based on any one of the following:
    • Physical Disability: Requires some assistance with at least one of the six Activities of Daily Living due to physical impairment
    • Moderate visual impairment or worse in the better eye
    • Moderate hearing loss or worse in the better ear

*If your condition does not fall under the categories listed above, you may still approach SG Enable for a discussion.

How to Apply


1. Prepare supporting documents

1. Disability Verification Form (DVF)

To be completed by a relevant registered healthcare professional.

You do not need to submit the DVF if you already have a verified disability status.

2.  Fitness for Employment and Rehabilitation Form

To be completed by a relevant registered healthcare professional.

3. Hospital-to-Work Application Form To be completed for email/physical submissions only. You do not need to complete this form for online submissions.
4. OneMSF Omnibus Consent Form If you have already provided Omnibus Consent to MSF/SG Enable in the past, you do not need to fill up the Omnibus Consent Form again.             
 5. Copy of NRIC Front and Back  


2. Submit your application in one of these ways:

  3. Approach SG Enable’s Information and Career Centre, located within the Enabling Village, for assistance
Upon the submission of all supporting documents, SG Enable will take up to 14 working days to process your application.

For healthcare professionals making referrals: 

A representative from SG Enable will contact the referral source/potential candidate for further discussion and to retrieve the necessary documents.

Frequently Asked Questions (FAQ)


Q: Do we have to pay for the programme?
A: There are no charges for the case management, job matching, and job support components of the programme. However, if additional rehabilitation services are required, there is a fee subjected to means testing.


Q: What is the duration of the programme?
A: The duration of the programme is one year. Persons with disabilities who require additional support beyond this will be assessed on a case-by-case basis.


Q: What types of jobs are available?
A: We have a variety of job positions in the public and private sectors. You can browse the job opportunities available on SG Enable's Disability Employment Jobs Portal. The portal is updated regularly with new opportunities. Note that Singpass is required to log in.


Q: Can I join the programme if I am still going for therapy?
A: Yes. You can continue to go for therapy while on the programme. A case manager will work with you to understand the frequency of your appointments and discuss with employers for customised arrangements if you are shortlisted for a role.


Q: Can I join the programme if I have a job?
A: Yes. We support both jobseekers and employed individuals in their transition back to work. If you have a job, we can provide on-site job support for you and your employer. This may include making changes to the work environment or work processes, subject to the employer’s decision, to enable you to perform your job duties.  


Q: What is the difference between the Hospital-to-Work Programme and SG Enable’s Job Placement and Job Support Programme? Which one should I sign up for?
A: The Job Placement and Job Support (JPJS) Programme provides persons with disabilities with employment assistance. The Hospital-to-Work (H2W) Programme is targeted at supporting persons with acquired disabilities. The transition for these individuals can be challenging, so on top of general employment assistance, H2W participants receive personalised career re-assessment and rehabilitative services if needed.
